(FP-33C) Motorvehicle Fire Report

Requirements for a Motor Vehicle Fire Report (FP-33c)

When the OWNER OF RECORD is seeking an FP-33c for a burned/recovered motor vehicle report, the following information shall be required in accordance with MGL Chapter 266, Section 29B.

STEP 1 - Required Documentation:

Prior to requesting an appointment with the Fire Department, the owner of record must gather and have in their possession all of the required documentation listed below:

  • Driver’s License of the vehicle owner
  • Driver’s License of the vehicle operator at the time of fire (if not the owner)
  • Copy of the vehicle registration'
  • Insurance Company Name, Agent’s Name, and Phone Number

If the vehicle was stolen, the following additional documentation is required:

  • Police Department Stolen Vehicle Report
  • Recovered Vehicle Police Report

STEP 2 - Request an Appointment:

Once the required documentation has been gathered, in order to process a Commonwealth of Massachusetts Department of Fire Services Burned/Recovered Motor Vehicle Report (FP-33c), the owner of record of the motor vehicle must contact the West Springfield Fire Department - Fire Investigation Unit, to request and schedule an appointment.

Please make sure to include in the email or message:

  • Your name
  • Contact phone number and best time for a return call
  • Date of the incident
  • Location of the incident
  • Vehicle Type

Once a request has been made, a member of the Fire Investigation Unit will contact you by email or by phone to confirm the appointment date and time.

STEP 3 - Your Appointment:

  • At the time of the appointment the vehicle owner must bring all of the required documentation listed in Step 1 above.
  • If someone other than the owner was operating the vehicle at the time of the fire, the vehicle operator must also attend the appointment and provide a statement (if needed) to the fire investigator.
  • The location of the appointment will be Fire Headquarters, located at 44 Van Deene Avenue, West Springfield, MA 01089.
  • Please park in the parking lot on the north side of the fire station. Please do not block the apparatus bays of the fire apparatus and ambulances.
  • You will complete the FP-33c form in person at the time of the appointment. The Fire Department will provide the form for you.
  • The Fire Investigation Unit may require additional information before the FP-33c form can be released.
